Submit your Story
Purpose
We are asking you to write or to record a short case study about your story and experience at/of Restore. The is an opportunity for you to show what you’ve achieved, how far you feel you have come, the impact that Restore has had, and how you’re doing now.
You can share as much as you are comfortable with. There are some questions below which might help you to think about what you want to say, but you don’t need to stick to these entirely.
We hope to include stories which will help us to demonstrate the impact of our work for people at different stages of their recovery journey.
Where will the story/video be used
We will include the story/videos on our website, and our YouTube channels, and will promote them on our social media accounts (Twitter, Facebook, Instagram) at periodic intervals. The digital files will be stored on the G:Drive, which is where the majority of our records and work are captured at present.
Your right to anonymity
You can choose to remain anonymous if you wish. If you want to do this, we can make sure we do not disclose your name on the story/video. If you choose to submit a video, we can further protect your identity by using your voiceover over some images instead. This is entirely at your discretion.
Permission
Restore requires your signed consent to use your story/capture your likeness on video. Signed consent forms will be scanned, digitised, and loaded to the G:Drive with your story/video.
If you choose to enact your right to be forgotten, or at any time change your mind, we will ensure that the Consent Form, story, and video are destroyed from all sources.
